The essential form to complete. You need to fill this in and give it to your captain or one of the commitee members (see contacts)to ensure that: 1) You are covered by the RFU player insurance policy 2) You can appear for the 1st XV in competitive games Get it done people and make sure new players are told to come here and to fill it in.
