Player Registration - 2012 -2013
All Members are required to complete online registration form which can be found by visiting: Registration
The Registration Fee for the 2012 - 2013 season is £60 per player.
The appropriate annual payment and submission of this Registration Form entitles your child to train and play with the club. For Youth age groups, this includes mid-week training and Sunday sessions. The Club endeavours to provide food for both Mini and Youth squads after home fixtures. There will be a discount of £20 off the full membership fee for any additional siblings. The club Equity Policy caters for reduced membership fees on a case by case basis, contact the Membership Secretary for full details.
For new player membership please provide two passport size photographs, named on the back and forward to your age group Parent Rep.
Medical Consent: Where a player is Under 18 application for membership implies that parents/guardians agree that if a child requires medical treatment during a club activity and it is not possible to contact said parent/guardian, the person in charge of the activity is authorised to give consent.
Photography: Where a player is Under 18 application for membership implies that parents/guardians understand that images of players/teams may be reproduced to promote an event or publicise the club in electronic or printed form. Any parent/guardian not wishing for images to appear should notify the club in writing.
Data Protection: Information supplied will be held on-line for use of the club only. All members are free to view personal information upon request.
