The main aims of Derwent Hockey Club are; - to play hockey of a high standard, - to be respected for our hockey, - to be known as a warm, friendly club where everyone feels welcome. Please read this Player Agreement and Senior Code of Conduct to be aware of what is expected of you to help the Club achieve these reasonable aims.

In order that we can fulfill our affiliation obligations, help us to qualify for funding claims and to update the player database, please print, complete and return a copy of this document at your earliest convenience. If you have any questions or issues, always feel able to contact your usual captain or a member of the committee.
Certificate of Insurance

Please review the attached file to be aware of the level of insurance cover that the Club provides. If you are concerned about attaining additional cover for personal injury and/or loss of earnings, please speak to an Insurance Agent/Financial Adviser. There are people within the Club who can help you with this.
Standing Order Form (Juniors)
To pay your annual subs and match fees via Standing Order, please print and complete this form and then forward it to your Bank before 15th September. A "Junior Member" is a player aged 16 or under.
Standing Order Form (Seniors)
To pay your annual subs and match fees via Standing Order, please print and complete this form and then forward it to your Bank before 15th September. A "Senior Member" is a player aged over 16, not in full time education and in employment.
Standing Order Form (Intermediate)
To pay your annual subs and match fees via Standing Order, please print and complete this form and then forward it to your Bank before 15th September. An "Intermediate Member" is a player aged over 16 AND in full time education or unemployed.
