Six Nations 2012 Application
2012 6 NATIONS INTERNATIONALS APPLICATION DETAILS Applications are invited from Members for the 2012 6 Nations Internationals. In order to apply for tickets, please send me a completed application form, which is attached here. Completed application forms should be sent to me at Sovereign House, Sovereign Street, Leeds LS1 1HQ and must be received by me by not later than 25 November 2011. Applications that are not on the prescribed form, or which are received after that deadline, will not be accepted. David Handy

If you cannot open it, please go to www.adobe.com to download the free acrobat reader. It has electronic forms, so you can complete it on your computer rather than hand-writing it if you prefer.
