RFU Youth Registration Form
All Junior/Mini players must complete on of these forms and provide 2 passport photos. When you have completed your form, please pass to your coach/manager or hand to Paul Kilshaw or Ian Milligan
Consent Form for Playing out of Age Group
Private document: Supporter, Player, Parent, Club Member, Staff, Coach, Team Admin, Webmaster
This form is to be signed by the parent and player where the coaching staff have agreed for players to play up one age group. It must be countersigned by the Safeguarding officer before the player is involved in any games.
