It is important that the club keeps up-to-date records for all players and volunteers.

The filling in of this form is mandatory, to ensure the club is complying with all the current safeguarding regulations.

**Note - Youth members should fill in a paper contact form, not this one - copies of the form are available from the Youth Committee.**

To try and make the process as easy as possible, both for players/volunteers to give us the information, and for us to process the information, we have an online form for people to fill in. It's very simple to use, and will only take a couple of minutes to fill in your details.

It works on smart phones (tested on Apple, Android and Windows phones) and tablets, as well as on a laptop/PC, so it can be filled in from anywhere with an internet connection.
