Thank you to everyone who has so far filled in registration forms for the Home Counties Premier League; unfortunately I didn't realise (eye test booked for next week) that we also need copies of your passport photo (or other photo ID with a copy of birth certificate).

Can you please make a copy of either document and get to me or Holey ASAP so we can complete the registration process.

Either of us should be at the club Tuesday or Thursday night. If you can't make a copy, please bring your ID and I'll be able to make copies and return the documents the same night.
