Please fill out this one-page form and bring it along when you join. If your details change (contacts, address, medical information, etc) - then please pass an updated form onto your coach or captain. We will keep your information confidentially

East League need this to be filled out at each match by the captains of Ladies 1st and All Men's teams. ALSO Don't forget to print off one the SNHC Match Fees sheet below to collect your teams match fees.

For Captains to summarise Match Fees paid in to our bank, and to reclaim any necessary expenses - bank details on the sheet. By paying fees directly into the club's bank, you really help our cashflow. You can pay in each week or collect a few weeks together, and then send in this 'Log' to help the Treasurer identify amounts paid in. Thanks
