Please also enclose your cheque made payable to SURFC for the sum of £25.00. Junior Player Membership can be obtained through Paul Coode (Junior Sec) and Paul can be reached on 01792 516532.

Order form for NEW 2011 Senior XV Gilbert Storm Jacket. Order forms to be returned to 1st XV Team Manager Roger Gadd.

Order form for the New design Club Supporters Shirt for 2011 and onwards. Please download and follow instructions on form to place an order. Please also be aware of time it can take in the usual course of events for delivery of your order due to a monthly batch order being placed as the normal plan for ordering / delivery.

Ticket application form for Home Ospreys Matches 2011-2012 Season. Completed form and Money (cash Only) to be sealed in an envelope and placed into locked Ballot box behind the bar in club at least 7 days before match start. NO LOOSE FORMS ACCEPTED.
