• UNDER 18 PLAYER CONSENT: It is a requirement of Jersey Hockey that parental/guardian consent is provided for participation,
transportation, medical details and photography. Jersey Hockey adheres to England Hockey’s Safeguarding Young People Policy Procedure and Guidance which is available to view online at www.englandhockey.co.uk/safe.

• Jersey Hockey adheres to the England Hockey policies for inclusion, diversity and compliance (including the Code of Ethics and Behaviour) all of which can be viewed at:

• MEDICAL: In case of emergency and as part of Jersey Hockey’s responsibility to its players, ALL registered players are required to complete the medical information section as accurately as possible.

• PHOTOGRAPHY: It is impossible to control photography by external parties. There may be times, however, that photographs and/or footage may be taken during matches and training sessions by approved agents and/or officers of Jersey Hockey. Such images shall only be used for publicity/training purposes and Photography Policy consent is sought for your child to feature in such photos/images. The consent only grants approved agents the right to use the images resulting from the photo/film shoots. This includes any reproductions or adaptations of the images for all general purposes, e.g. local newspapers, local magazines, other promotional articles (including flyers), for our sponsors’ requirements and Jersey Hockey’s website.

• TRANSPORTATION: Jersey Hockey adheres to the England Hockey guidelines on transportation of under 18 children. These guidelines can be viewed at:- http://www.englandhockey.co.uk/page.asp?section=1785&sectionTitle=Best+Practice+Guidance.

Parents/guardians are asked to ensure that arrangements are made for your child to travel to and from training sessions and matches. We also need to know if you have made alternative arrangements for your child to be the responsibility of a person or person who are not mentioned in this Registration and Parental Consent Form 2020/2021..

• INSURANCE: Accidents and injuries can, and do, happen on the hockey pitch. If your child unintentionally causes accident or injury to another person then, subject to the facts of the incident, there is cover under the Public Liability insurance taken out by Jersey Hockey on behalf of all its members. • DATA: All personal data will be kept in a secure database with access restricted to authorised persons for hockey purposes. Consents and Declaration

I have read the information as set out in this Joining Letter/Registration and Parental Consent Form. I fully support the spirit of the document and agree that the player named above will abide by Jersey Hockey’s policies as set out above.

I consider the player named above to be physically fit and capable of full participation and agree to notify the Jersey Hockey Administrator (contact@jerseyhockey.co.uk) of any changes to the medical information provided. In the event of injury, I give my permission for the team managers/coaches appointed by Jersey Hockey to obtain emergency medical treatment.

I agree that the consent given in this Registration and Parental Consent Form will be valid for the relevant hockey season and will be used for all activities in which my child participates within Jersey Hockey. I will inform the Jersey Hockey Administrator (contact@jerseyhockey.co.uk) of any important changes to my child’s needs and also of any changes to our address, email or phone numbers given.

I give consent for the player named above to feature in the photos/images stated in the Photography policy above.

I have read the England Hockey Safeguarding Young People and Inclusion, Diversity and Compliance (including the England Hockey Code of Ethics and Behaviour) Policies as noted above and I, and the player named above, agree to abide by them. I and the player named above understand the expected levels of behaviour for everyone involved in hockey.

I understand that all data provided will be stored by Jersey Hockey in line with its Privacy Policy.

I confirm that all the information provided in this Registration and Parental Consent Form is true and accurate to the best of my knowledge.

I recognise that spaces within the MINIS Development Group may be restricted and that a waiting list will be employed if necessary.
