Please either enter a "1" if available that week or a "0" if not. This will be used at team selection meetings every week so the importance that everyone completes it could not be over emphasised.

Page Two is for everyone to enter their contact details, which again is of great importance for selection and our captains.

I will take information added to this and enter it on the master copy I hold on my laptop, and it will be that copy that will be sent to the captains each week
