1. Membership Forms for All

For Club membership please download and complete the following forms attached below:

• Club Membership Form
• Direct Debit Form*
• Photo and Medical consent

*The preferred method of payment is by Direct Debit (DD) and a DD form can be downloaded...see below . Once completed PLEASE DO NOT send the DD form to your bank as a unique Club Reference Number needs to be added by the Club. Simply return all completed forms to your Team Manager or Membership Secretary by hand or email: membership@lutterworthrfc.com

For new membership of Players under the age of 18 It is really important the the Youth Affiliation process is completed BEFORE/AS SOON AS you hand over your membership form to your team manager. We will be unable to process your membership until this has been completed.
