3. DBS Checks


All volunteers who are working with children at the Old Brods need to have an RFU DBS (Disclosure and Barring Service) check. Forms are available from Jason Gomersal, Club Safeguarding Officer, who will also check your identification documents and sign off the form ready for submission to the RFU.

In order to complete the form you will need to bring some of the following documentation:

  • If ONE document produced from group 1, then a further 2 documents from either group 1 or 2
  • If no document from group 1, then 5 documents form group 2

GROUP 1 DOCUMENTS
Valid Passport (any nationality)
UK Driving Licence (if a photocard then the paper counterpart is also needed)
Original UK Birth Certificate
UK Adoption Certificate
Valid National Photo Identity Card (EU countries)
UK Firearms Licence
HM Forces ID Card

GROUP 2 DOCUMENTS
Marriage/ Civil Partnership Certificate
Non-original UK Birth Certificate
TV Licence (less than 12 months old)
Certificate of British Nationality
Council Tax Statement (less than 12 months old)
UK National Insurance Number Card
Benefit Statement (child allowance / pension) (less than 12 months old)
UK P45 / P60 statement (less than 12 months old)
Valid Vehicle Registration Document (V5 or V5C Only)
Court Claim form (less than 12 months old)
Addressed Pay Slip (less than 3 months old)
UK NHS Card
Connexions card
UK Work permit / visa (issued less than 12 months ago)
CRB disclosure certificate (issued less than 12 months ago)
Financial statement (pension, ISA, endowment) - (issued less than 12 months ago)
Bank or Building Statement - less than 3 months old
Mail order catalogue statement - less than 3 months old
Credit Card Statement - less than 3 months old
Mortgage Statement - less than 12 months old
Exam Certificate (GCSE, NVQ)
Valid Insurance Certificate - issued in last 12 months
Utility bill - less than 3 months old

You also need your address history for the past 5 years, with dates and postcodes.
